I still haven't figured out how any of them lived long enough to get really good at it, must have had a lot of non-lethal fights.

A Gladiator could, by rule, raise either hand with their middle and index fingers together and the remainder of their fingers clenched to the editor of the match. This is to signal "Missio", ceding the match and plea for mercy.
The editor would have the choice of whether the gladiator would live or die.

Yes. In fact, gladiatorial combat was regulated. Dying in fights wasn't as common as the modern movies portray it.
