In the never-ending quest to band aid my swing, I have this problem too. It comes and goes and I've developed some bad habits. I've noticed that my alignment has crept more and more closed. Last week I corrected it and it felt like I was 200 yards open at address. Just that one adjustment "fixed" some of my out-in path. I guess my brain was overcompensating when I thought my alignment was fine.
Also, the glove drill someone mentioned helped out a lot.
I spent a lot of time adjusting my grip and that had the most affect. When I get too weak, I pull everything left and then I compensate to too strong and I hit missiles about 300 yards right with a push slice. It's a fine line but that's what has helped me lately.
