clean out the joints and install polyurethane sealant. If you leave them open water will intrude and damage will result

Is it easy to install or do I need to hire professional

that depends on quantity and condition of your joints (and the driveway joints)

Pull out the wood and clean the joint. Fill with sand leaving about 3/4" open. Top with a polyurethane about 1/2". then go back on top of the polyurethane with a very small size gravel that's as close to color of your driveway as possible.

Good plan here but I would do the following on the wet self leveling sealant:
-Sprinkle baby powder on it if you need to drive on it in 24 hours.
-If you can give it a few days sprinkle fine play sand (essentially masonry sand with no rocks) on it when wet.
It's critical you fill the void with sand or backer rod before caulking it with self leveling sealant or it will just seep in all the cracks and not fill the void.

I need to do this as well. If you have any kind of slope to your driveway, you need to seal those up. Just look at the Lowe’s or Home Depot website and search for “concrete expansion joints rubber” or something like that. You can buy products in rolls to fix this. The only thing you need to know is what length you need and the crack width.
