that code is for desensitizing resin. if your dentist placed a large filling, he was worried about you having sensitivity or requiring a root canal in the future. he placed the 'medicine' in an attempt to prevent you from needing further treatment. so you should thank him and call your POS insurance company and ask them to explain why they refuse to pay for treatment your dentist has deemed necessary.
Desensitizers under fillings and crowns
per CDT all liners and bases are included. Codes D9910
“desensitizing medicament” & D9911 “desensitizing resin”
codes are ”not to be used for bases, liners, or adhesives
used under restorations”

Usually it's 50/50
Not usually. Usually it's 100%.
$1500 is a pretty good PPO plan.
About 50% of folks with dental insurance never obtain services in a given year. They are helping your premiums stay low. Thank them.
Your insurance carrier does not determine your plan benefits. Your employer does. Get with HR and tell them you want a plan with a higher annual maximum and higher co-insurances. Hell, ask them for a $0 ded.
Insurance companies can do all that. We can write in free diamond grills too. Ask for it.

most insurance covers normal cleanings and yearly x-rays at 100 percent
now where it can vary is some of the minor stuff like fillings and whatnot...I believe mine is staying at 80 percent coverage for those things while others are 50%. Hopefully I don't have to worry about this, though.

Your dentist does not work for the insurance companies, nor is he employed by them. They do not list all the dental codes. If you want that you can order the ADA codes book "CDT 2015 for $59.95 and the CDT 2015 Companion: Help Guide and Training Manual $74.95 or the package together for $112.50

My dental is pretty good. Covers 2 cleanings a year plus xrays at 100%. Most other stuff like fillings, extractions, etc are covered at 80%. For $12.50 a month, I think it's worth it.

Ever since I got dental insurance, it's become really easy to tell how much services cost.
My dental visits are always seem to wind up equal to the maximum amount my insurance covers per visit/service.
