I don't know who started that, I remember Dr. X doing that when I was a kid, the premise was there was a weight in the boot, and when he "loaded" it by kicking his toe down on the mat, positioned it to the front of the boot, for a brass knuckles effect, very risky move, for it was an instant dq to get caught with a loaded boot

Definitely the Grappler, That shite was funny as hell. He would tap his heel several times on the mat and the announcer would say he is loading up the boot. In reference that his boot was loaded with lead then he would kick the opponent and he would fall out cold as ice.
