If the car still has a cigarette lighter, what in the Sam hell would you be connecting to? Or are you talking about a BT speaker phone?

Man. You gotta explain a little.
Do you only have an older radio that you need a Bluetooth FM transmitter to stream your phone to the radio ?

As you’ve probably picked up by now from this thread, the only thing a cigarette lighter socket is capable of doing is transmit electrical current. That’s it. If you’re trying to get music from your phone to play out of your car’s speakers, we’re going to need more information.

I'm pretty sure that his car stereo unit doesn't have bluetooth built in and the adapter (which has bluetooth) will receive and broadcast the stream to FM. Usually a frequency down at the lower end of the band.
The adapter gets its power from the lighter socket.

Depending on the vehicle, and unless it had a non standard dash opening, I'd considering upgrading the head unit. You'll get better reliability, more features and improved sound quality.
I recently replaced the old DIN sized head unit in our spa to one with Bluetooth. Now I can stream the broadcast from our outdoor FireTV to the spa. It was less than $100 and took me less than 1 hour to install (but I didn't have to deal with a car dash). Best buy and other big box stores usually do simple installs for free on head units that cost more than $200.

I have a 2016 F150, Sync took a shite so I use this Monster brand FM Modulator with zero issues.
