I know of two instances where the perp(s) got caught:
1. Minority students sitting behind me in an engineering mid-term exam. The whole class could hear them discussing the questions out loud. The professor found out, took their exams, and reported them to the Dean. Next class, they were back in their seats.
2. A fraternity brother whose father was a relatively known lawyer attempted to steal a copy of a final exam for his gf. Somehow he caught wind that another Section of the class was going to see the same exam as her, so he attended the earlier section’s exam time slot, then attempted to just walk out w a copy of the exam. The professor attempted to stop him so he threw a chair, etc. and was ultimately caught. He was kicked out of LSU. His father helped him get a new SS number, then he re-enrolled the next year like nothing happened.

We had older siblings that took the same classes a few years before us that had stuff on a cd I think? Whatever it was, it was the fact same assignments that we were assigned a few years later so we used their old materials.
I think I snuck in a little index card in my TI83 calculator one semester with math formulas on it.

Friend of mine got caught cheating in math class. Teacher kicked him out of class which led to an F. Which led to him dropping out of school. No worries he owns a bunch of TV stations in southwest Missouri. He is doing fine.

I became friends with a track athlete in one of my earlier years in the 80’s. We kept in touch throughout the college years. I was taking a computer science class my second to last semester and he was enrolled in it also. He was a senior taking 12 hours to graduate. He had good grades but for some reason could never grasp the language. He failed the course the first time and made a D the second time he took it. Needed a C
He had a friend take the class under his name. Class had about 35 students in it. The friend went all semester to class and had him an A going into the final.
During this time very few professors required ID’s to take exams but….On the board for the final the professor had written that you have to show your ID when you turn in your exam. His friend had no choice but to leave. The professor already knew the friend wasn’t him but let this go on all semester. Of course the professor failed him and had him kicked out of school needing 3 hours to graduate.
The friend never got caught as they couldn’t figure out who he was.
He had to fake sickness on graduation day so his parents wouldn’t show up. I believe he had to sit out a year and then he ended up taking the class again. We lost touch after I moved.

Almost all Chen engr classes in the late 1967 period were open book. I had a lot of handwritten prompts written inside the front and back cover. While the info was available in the open book, I saved a lot of time by not having to search.

Most my upper level ChemE classes in early 2010s were open book as well. Professors didn't care because most people didn't finish the exam and if your had to rely on the book for more than just a couple of bookmarked formulas, then you were maybe going to finish 50% of the exam.

In large classes. I used to get to class super early and write stuff in pencil on my desk. Or really light inside my blue book. For smaller classes. I used to write info really small on slips of paper and slide them inside a clear Bic Pen

It was his last class and the professor divided them into groups of four and assigned four projects. The professor said “Every project is a group project, and every student must work on each of the projects.”
They decided it would be much less work for each student to do one of the four projects and put all their names on it. The first three group projects went well but the fourth was a disaster.
The fourth team member got busy with planning his new job and place to live after graduation and ran into problems with the project. He borrowed a project from a student who took the class two years previous and basically just copied it.
The professor caught the copy and gave all four group members a zero and failed them all. The guy who copied the project was kicked out of school and the other three had to cancel their jobs and return to school one more semester to retake the class.
