The case I described yields many possible examples, and here is one:
1. Undefeated or one-loss ACC champion
2. One-loss Pac-12 champion
3. One-loss Big 12 champion
4. Two-loss SEC champion LSU
5. Two-loss Irish
6. Two-loss B1G Champion

LSU gets in in this case. No doubt.
