No, that's not the reason...then she would look ugly because all he can see is the inside regardless if he knew them before.
She still looks hot because her inside is just as hot.

Im pretty sure the hot neighbor would be less attracted to him after he was dating the 400 plus pounder. That part always bothered me

Yeah. I thought (with women) he started seeing them as they appeared on the inside (ugly/hot) and that since the neighbor Jill was the one woman we knew before hand (and she stayed the same) it was the movie telling us she was a really good person. She's the only sample size we have...as all the other girls in the movie are after the fact.
Or maybe not...taking the script literally...
