Buddy lost his leg. Moved the lift when he was up. #1 thing not to do. I'm surprised the lifts don't automatically prevent this.

Nope, but I did a bunch of sketchy shite on this thing back in the day.

Yeah it's f***ed up and guys can die.
LINK .
Few years ago a kid at Notre Dame died when his blew over as he was filming football practice.
On a larger scale, scissor lifts and boom lifts are scary as heck. Guys get on and don't know how to operate them, think it's just basically a golf cart or lawn tractor.
I've seen a number of people get hurt this way and did some dumb risky stuff earlier in my life, too.

i worked for the "premiere commercial builder" in Louisiana in college. They had a fatality due to a scissor lift falling over. Needless to say the use of them was highly observed after that.

Was the mall superintendent building Bonita Lakes Mall in Meridian, MS in 1997. One of the drywall framers was up on a 26' scissor lift in the concourse when he drove a wheel into one of the floor openings for the planters. The cover over the hole hadn't been replaced while the landscapers were working on it. (They had walked away from it for all of two minutes.)
The lift went over but the framer was able to grab a sprinkler line over head to keep from coming down with it. Of course, he couldn't hold on long and had to drop 22' to the concrete below. Injuries sustained were a sprained ankle, bruised knee, mild concussion, and a laceration to his scalp when his head hit a stack of sheetrock next to the opening.
He would've had a great case for workmans' comp had he not just smoked a big fatty at lunch about thirty minutes earlier.
No OSHA fines issued, fortunately, and even more fortunately no one was seriously injured. I recall writing up the accident report on that one. Oddly enough, scissor lift safety had been the topic of our site-wide safety meeting the previous morning. Some guys just don't pay attention during those boring-arse safety meetings, huh?

Remember sometime around maybe 1996 it happening during the halftime show of East Ascension's Homecoming game.
Basically what happen is the band was playing and someone went up on a scissor lift with a gigantic banner, 40' high or so.
Wind gust, pushed the whole thing over.
Band never saw it happen as it was behind them. Never missed a note. Luckily it fell backwards or it would have been into the band.
First responders in the crowd took off running through the band.

Was she wearing seatbelt? Did she perform inspection prior to operating the forklift?
