It's quite possible that they were improperly made grenades.
If they were poorly cast, they may not fragment correctly and would fail to produce a large amount of shrapnel.
Cook off a couple of M67's in that same scenario and nobody walks out of that room!

Twitter is wild nowadays man. I opened the app the other day to see who the Gamecocks landed from the portal and first thing I see is a video of a guy getting gruesomely smashed by a falling piece of debris.

What I read was 1 dead, 26 injured, 6 in critical condition. Must have been something like flash bang grenades and not fragmentation grenades or there would have been a lot more deaths.
