Xerox - they developed the mouse and the graphical user interface. Basically, they could have been Apple AND Microsoft rolled into one and completely dominated the computer revolution, but they didn't realize/couldn't capitalize on what they had and now they're a copy machine company in an increasingly paperless world.
AOL - everyone used them to get online in the dial-up era and they either didn't foresee broadband or didn't do anything to re-position themselves or use their money to develop new technology and now they're merged with Time Warner which is viewed as one of the worst most worthless acquisitions ever because they contribute nothing to the modern world.
Research In Motion (Blackberry) - had the smartphone market on LOCKDOWN and a decade later they barley exist

Blockbuster.
They had a chance to purchase Netflix for practically nothing somewhere around 25-50 million. Now there is no blockbuster and only Netflix.

AT&T was bought out by SBC, one of it's RBOC's in 2005. Being bought out by one of your divestitures could imply poor management/strategic planning leading up to the buyout.
Most of what you know about AT&T today is because of the changes brought forth by SBC.

Blockbuster got conned into a deal with Enron. Enron made them believe that they had the infrastructure in place to deliver streaming video on demand over the internet. Not only did Enron not have the capacity, they DIDN'T EVEN HAVE A BROADBAND NETWORK.
