This is how I grew up, also. I’d be interested to know what percentage of American households this still occurs in. I’d guess <5%.
To the question, once every couple of months.
I dont know but for anyone with a family, it's a good habit.
I dont see my brothers that often, but we were all down at the beach for Father's Day. Did a fishing trip one morning. Still cooked and ate together. Didnt even go out to eat while we were there. So, it's something that will stay with you.

It was usually a special occasion to go out to a restaurant. My mother didn’t start working full-time until I was in my teens, so she always fixed suppers. Fridays nights we would get to watch TGIF and we’d get take-out. My parents were strict about TV/video game usage, we were usually made to go outside and play, but Friday night was the one time when we were allowed to veg out for about 2 hours.
My wife and I these days go out to eat way too much these days, but she works a lot and so do I. For us, it’s not so much the cooking as it is the clean up. Neither of us want to do the dishes afterward, but we also don’t want a dirty kitchen. The solution? Let’s go out to eat.

Not often at all, and when we did, it was more of the Frostop, Angelo's (before Rocky & Carlo) broke out on their own or some other mom & pop type local restaurant.
If up at an aunt's house uptown, it was typically Frankie & Johnny's, Parasol or the now defunct "Pete's" in the 4200 block of Magazine.

All the time and I hated it.

Rarely. My dad occasionally took us to Dons Seafood on airline. My mom made us thank him for taking us out to eat. I find that kind of odd .

Full meal? Rarely. Once or twice a year.
Coffee shop with something from a sweet roll to an egg breakfast? Semi-regularly.

Every single Friday night. That was the thing.
Fast food is also probably once every other week or so.

Every Sunday after church, and probably one other time per week

Not much, we’d go to the Cowpen once a month or so. My mom is a helluva cook, for a 5’3” 115lb woman, she can throw down in the kitchen like a fat black woman.

Once a week. If we got straight As then we got to go to the fancy Italian restaurant in South TX. Can’t remember the name but I’m sure it was probably a step up from Buca de Pepo or whatever it’s called.
Now, we don’t cook any meals Fri-Sun.

Hardly ever. Special occasions we would go to El Chicos, Panchos, or Western Sizzlin’ in Bossier City. Super special occasion is Ralph & Kacoos.

JC’s on Veterans every Friday night, this would’ve been early to mid-70’s. It eventually became Ralph and Kacoo’s.

The only franchise restaurant in our parish was a DQ, and I recall us getting tacos from the drive thru a few times. And I remember one meal at a local BBQ place.
Otherwise, there was annual trip to Pizza Inn for my birthday, and we hit Pancho’s and McDonalds a few times when we were in Shreveport for 4-H/State Fair or such and couldn’t get home in time for dinner. We were told on those McDonalds visits what we could have: regular hamburger, split a small fry with your brother, small coke.
That was it until I made my own money from about 13 on. I remember going to LSU for 4-H short course in summer and going nuts eating at Mr. Gatti’s, IHOP, and McDonalds (order anything I want!). You would go home and brag about that like you ate at Le Bernardin or The French Laundry.
We weren’t poor. My parents were just frugal, and there weren’t many good options for eating out where we lived.
